ABSTRACT
Objective
This study aimed to evaluate nursing care plans in terms of nursing diagnoses, nursing interventions, outcomes of interventions, and factors affecting these outcomes.
Methods
The study was conducted in two phases between May and July 2024 with a total of 30 nurses working in the internal medicine, surgical, and intensive care clinics of a teaching research hospital. In the retrospective phase, the nurses’ handwritten 5-day care plans were examined. In the qualitative phase, interviews were conducted with the same nurses using a semi-structured questionnaire. Descriptive statistics were used to analyze quantitative data, and content analysis was used to analyze qualitative data. Ethical approval and necessary permissions were obtained for the study.
Results
Nurses most frequently identified infection risk, anxiety, and pain diagnoses, determining these diagnoses based on the patient’s medical history, examination findings, and physician orders. It was found that the majority of interventions implemented for these diagnoses resulted in a reduction in problems. It was determined that the most important reasons for the interventions not yielding positive results were insufficient manpower and physical resources, and that nursing management supervised the care plan and provided educational support.
Conclusion
Standard nursing diagnoses are used in the implementation of care plans. A significant portion of interventions carried out in line with diagnoses do not yield full results. There are various reasons for this situation, primarily related to labor and physical conditions. Nursing care plans are monitored by nursing services management, and nurses are provided with training support in this regard.
